To register, just contact the Régie by telephone and ask to be sent the appropriate form.

If you are a Canadian citizen, you must provide the Régie with the original of your birth certificate or photocopies of both sides of your Canadian citizenship card. If you are a Canadian citizen but were born in another country, you must submit photocopies of both sides of your Canadian citizenship card or a photocopy of your Canadian passport.
